UTI project: Urinary Tract Infection management in General Practice Out-Of-Hours Care
Urineweginfecties zijn de meest voorkomende reden waarmee vrouwen bij de huisarts komen. Veel mensen krijgen hiervoor antibiotica, terwijl dat niet altijd nodig is. Te veel gebruik van antibiotica kan zorgen voor resistentie, waardoor deze medicijnen minder goed werken.
Doel
Het doel van dit project was enerzijds begrijpen waarom huisartsen zich in de helft van de gevallen niet aan de richtlijn houden. Daarnaast was het doel om de waarde van nieuwe sneltesten te onderzoeken die dit kunnen verbeteren voor de dokter en patiënt.
Werkwijze
Onderzoekers combineerden kwantitatief en kwalitatief onderzoek. Ze onderzochten de waarde van 4 nieuwe sneltesten naast de urinekweek als gouden standaard middels een diagnostische studie op de huisartsenpost. Ook keken ze met vragenlijsten en interviews waar rekening mee gehouden moet worden om de nieuwe sneltesten te implementeren.
Daarnaast onderzochten ze hoe patiënten, huisartsen en doktersassistenten omgaan met klachten en behandeling. En waar rekening mee gehouden moet worden om het volgen van de richtlijnen te verbeteren.
Resultaten
Eén sneltest, Molecular Culture, bleek nauwkeurig en snel bacteriën te kunnen opsporen. Deze test is veelbelovend, maar nog niet geschikt voor dagelijks gebruik in de huisartsenpraktijk.
Veel patiënten, vooral bij een eerste infectie, zijn bereid om eerst af te wachten of pijnstilling te gebruiken. Met goede uitleg, samen beslissen en zorgvuldig testen kan onnodig antibioticagebruik worden verminderd. Dit verbetert de zorg en helpt antibiotica werkzaam te houden.
