ACT: Appropriate Care for people with chronic diseases or advanced age in their care Trajectory

Adults with chronic illnesses and people of high age (80+) often have changing care needs, which nursing professionals cannot always fully identify, partly due to time constraints and limited information exchange. 

Aim

We aim to develop care approaches that truly fit the individual, improve quality of life, and strengthen both the role and job satisfaction of nursing professionals. Our focus is on bridging the gap between what really matters to people and what is monitored, delivered, and measured in daily practice.  

Approach

In work packages, we: 

  1. map care needs
  2. work towards appropriate care
  3. address appropriate care in the greater context of health and care

Relevance

Care that adapts to changing needs improves quality of life, helps prevent problems from worsening, makes work of nursing professionals more rewarding, and supports better and shared decision-making.
