Aim
The aim of this project is to develop a allied healthcare guideline Self-management.
Approach
The allied healthcare guideline Self-management focuses on adults with chronic conditions and dealing with a chronic condition, whereby attention is paid to optimally integrating the condition into daily life and maintaining self-control through self-management strategies. Another focus point is the organisation of care regarding the support of adults with chronic conditions from allied healthcare providers, in particular occupational therapists, skin therapists, speech therapists and dieticians. In this support, sufficient attention is paid to cultural responsiveness and comprehensible communication with the patient
