CO-ACT – Samen ontwerpen van adaptieve zorgtransformaties voor vergrijzende gemeenschappen
Europe’s ageing population, rising healthcare costs and workforce shortages challenge health and care systems. Although effective healthy ageing interventions exist, their impact is limited by implementation barriers.
Methods
This project addresses these gaps through tailored, context-specific approaches, using participatory methods, service and implementation design, and cross-regional collaboration to strengthen person-centred care pathways.
4 interconnected work packages build on evidence-based interventions, tested in living labs in Finland, Ireland, Portugal and the Netherlands. Local health and care ecosystems will be mapped to identify stakeholders, resources and barriers, guiding targeted adaptations. A shared repository of solutions will be developed and refined, supported by health equity assessments to improve accessibility.
(Expected) results
Using a systems approach, the project will support integration, scalability and transferability, and share results through a knowledge exchange platform.
