AT-HOME studie: Vancomycine thuis: modelgestuurde dosering versus traditionele zorg

For patients with severe resistant infections, H-OPAT is offered in Europe: intravenous antibiotics at home under nurse supervision. However, patients often need to visit the hospital for traditional therapeutic drug monitoring (TDM), causing travel time, costs, and discomfort. Moreover, TDM-based dose adjustments are often suboptimal.  

Aims 

The AT-HOME consortium aims to improve H-OPAT by integrating model-informed precision dosing (MIPD) and dried blood spot (DBS) sampling. DBS is a simple finger-prick method that patients can perform themselves after training. MIPD uses validated models to optimize dosing based on patient-specific factors.  

(Expected) results 

This approach is expected to reduce hospital visits, improve therapy efficiency and patient satisfaction, and lower costs compared to current standard care. The goal is implementation in Belgium, Sweden, Spain, and the Netherlands, with further scaling across Europe
