Organising integrated care for vulnerable elderly people living at home

The number of elderly in the Netherlands will rise sharply, with >25% of the population expected to be 65+ by 2040. This will pressure care services, necessitating a tailored, integrated approach to vulnerable elderly individuals. Essential interprofessional collaboration between healthcare providers and the social domain creates complexities in roles, responsibilities, and information exchange. 

Aim and approach

Regional GP organizations like Dokter Drenthe are reorganizing primary care for the elderly. Key questions include task differentiation, triage, patient self-management, and factors affecting collaboration such as payment structures and legislation. This project includes 4 Work Packages (WP). WP1 involves a survey and Delphi study among stakeholders to assess needs and roles. WP2 maps regional initiatives and outcomes in Northern Netherlands. WP3 examines a Dokter Drenthe pilot project using qualitative methods. WP4 analyzes the impact of pilot projects on outcomes used in WP2.

    General Practitioner care research

    We need more knowledge from research into general practice, and we need to disseminate this knowledge among GPs in order to maintain the current high standards of research and care. We therefore support academicization by funding PhD research projects for GPs training to become clinical researchers.
