STARS-Health

In Europe, 42% of people struggle with health literacy, particularly older adults, individuals with lower education, and rural residents. This increases chronic diseases and healthcare use, straining the system. Many older adults also face challenges with digital tools.   

Aims 

This project aims to create an integrated care system supporting caregivers in clear communication with older adults with Limited Health and Digital Literacy (LHDL) and their families, helping them use digital services, improve self-management, and reduce health inequalities. 

Methods 

The project will redesign the FindMyApps app to offer user-friendly apps and help patients monitor their health, while caregivers will be trained to provide better support.  

(Expected) Results 

This will enhance communication, boost patient satisfaction, improve caregiver job satisfaction, and ease pressure on the healthcare system, ultimately improving accessibility and equity in care.
