Strengthening Person-Centred SDM in Nursing Practice for Older Adults with Complex Needs

Shared Decision Making (SDM) facilitates nurses in supporting choices that align with the values of their clients. 

Aim

This project strengthens Shared Decision Making (SDM) for older adults living at home with complex care needs. By 2030, it aims to improve SDM in >30 care organisations, reach >500 nurses, and ensure that >1.000 older adults and caregivers feel heard, involved, and satisfied with their care decisions. By promoting self-direction and better use of technology, the project contributes to appropriate, person-centered care.

Approach

The project studies the effectiveness of SDM and adapts its tools for nursing practice, improves care transitions, and includes people with limited health literacy and diverse backgrounds. Across five work packages, the project develops and conducts research into measurement tools, training programs, decision aids, and inclusive strategies. 

Collaboration

The research is embedded in strong collaborations between education and practice, and supports policy, education, and daily care delivery.
