Supporting children’s with complex chronic conditions or palliative needs at home: HOME-CP2
This project aims to optimise coordination between primary and specialist care to improve home care for children with complex chronic conditions, by using technology that is tailored to their needs.
Methods
Caregivers will be provided with a set of home monitoring devices personalised to each patient's needs to perform the appropriate biomedical measurements at home. Healthcare professionals will develop a shared, individualised care plan for each patient, ensuring that care is participatory, focused on the patient's needs and comprehensive across different levels of the healthcare system. Coordination between primary and specialist levels will be facilitated by a cloud platform.
(Expected) results
This is expected to reduce the number of emergency consultations, admissions and average length of stay in hospital.
