Clinical prediction rules: towards a framework for meta-analysis, validation, and updating
Systematische onderzoek naar klinische predictiemodellen
Bij de behandeling van patiënten moet de arts vaak een diagnose stellen of een prognose geven. Hiervoor worden verschillende patiëntkenmerken of testuitslagen overwogen. Klinische predictiemodellen zorgen voor het combineren van deze patiëntkenmerken om diagnostische en prognostische kansen te schatten. Voordat een nieuw ontwikkeld predictiemodel kan worden toegepast in de praktijk moet het getest worden op een nieuwe groep patiënten. Wanneer de validatie mislukt ontwikkelen onderzoekers vaak een nieuw model voor deze nieuwe patiënten in zich te baseren op bestaande modellen. In dit project zal naar methoden gezocht worden om bestaande modellen te verbeteren en up-to-date te houden voor toepassing in de praktijk. Daarnaast willen de onderzoekers methoden ontwikkelen voor het combineren en integreren van bestaande vergelijkbare modellen tot één breed toepasbaar predictiemodel.

Bij de behandeling van patiënten moet de arts vaak een diagnose stellen (wat is de onderliggende ziekte?), of een prognose geven (wat zal de uitkomst van deze ziekte zijn?). Voor het stellen van een diagnose of schatten van een prognose wordt vrijwel altijd een combinatie van meerdere patiëntkenmerken en/of testuitslagen in overweging genomen. Klinische predictiemodellen zorgen voor het expliciet combineren van al deze patiëntkenmerken om diagnostische en prognostische kansen te schatten op een statistisch valide wijze. Voordat een nieuw ontwikkeld predictiemodel kan worden toegepast in de praktijk dient het gevalideerd te worden. Valideren bestaat uit het testen van het model op een nieuwe groep patiënten. Indien de validatie slechte resultaten oplevert, d.w.z. het model kansen oplevert die niet goed aansluiten bij de observaties in de nieuwe groep patiënten, ontwikkelen onderzoekers vaak een compleet nieuw model voor deze nieuwe patiënten in plaats van voort te bouwen op bestaande modellen. In dit project zal onderzoek gedaan worden naar nieuwe methoden om bestaande modellen continu te verbeteren en up-to-date te houden voor toepassing in de medische praktijk. Klinische predictiemodellen worden in toenemende mate ontwikkeld, wat leidt tot een overvloed aan modellen voor hetzelfde medische probleem. Zo zijn er bijvoorbeeld meer dan 60 modellen ontwikkeld om de kans op overlijden van borstkankerpatiënten te schatten. Dit project beoogt eveneens methoden te ontwikkelen voor het combineren en integreren van bestaande vergelijkbare predictiemodellen tot één breed toepasbaar predictiemodel. Tenslotte zal er worden nagegaan wanneer modellen teveel verschillen om ze te kunnen combineren.
