De combinatie van diabetes en slechte doorbloeding kan aanleiding vormen voor slecht genezende wonden. De behandeling van deze wonden is lastig. Als de behandeling faalt kan uiteindelijk een amputatie nodig zijn.
Hyperbare zuurstoftherapie (HZT) is een mogelijk effectieve, aanvullende behandeling, maar vergt dagelijkse behandelingen gedurende 6-8 weken. Het is daarom een intensieve en kostbare behandeling. Als HZT de wonden sneller laat genezen en/of amputaties kan voorkómen, is het mogelijk wel de kosten waard.

Dit project omvat een internationaal onderzoek om de effectiviteit van HZT en het aantal behandelingen dat nodig is om dit effect te bereiken te onderzoeken. Het onderzoek zal gedaan worden bij maximaal 544 patiënten met een diabetische wond en slechte doorbloeding.

SAMENVATTING • DOEL/VRAAGSTELLING Is hyperbare zuurstoftherapie (HBOT) (kosten)effectief voor de behandeling van ischemische diabetische voet-/beenulcera? • HYPOTHESE HBOT verbetert de weefseloxygenatie en daardoor de genezing van ischemische diabetische voetulcera. Hierdoor worden majeure amputaties (boven het enkelgewricht) voorkomen. • STUDIEOPZET Multinationale gerandomiseerde klinische trial met 4 studiearmen en geplande interim analyse (MAMS design). • STUDIEPOPULATIE/DATABRONNEN Patiënten met diabetes type 1 of 2, perifeer vaatlijden en een ulcus op de voet of het onderbeen. INTERVENTIE Patiënten in de interventiegroep krijgen 20, 30 of minimaal 40 sessies HBOT in aanvulling op de gebruikelijke wondzorg en overige behandelingen. • GEBRUIKELIJKE ZORG /VERGELIJKING Patiënten in de controlegroep krijgen de gebruikelijke (wond)zorg en (vaatchirurgische) behandelingen. • UITKOMSTMATEN De primaire uitkomstmaat is de frequentie van majeure amputaties na 12 maanden. Secundaire uitkomstmaten: kosteneffectiviteit, amputatievrije overleving, amputaties onder het enkelgewricht, wondgenezing en mortaliteit. • SAMPLE SIZE BEREKENING/DATA-ANALYSE Maximaal 544 patiënten zijn nodig om een verschil van 15% in majeure amputaties aan te tonen. Uit de interim-analyse van 272 patiënten zal blijken welke studiearmen stoppen wegens futiliteit of doorgaan omdat zij effect lijken te hebben. • KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Wij doen zowel een kosteneffectiviteit (KEA), een kosten-utiliteit (KUA) en een budget impact analyse (BIA). De KEA betreft de kosten per voorkómen amputatie. De KUA omvat de kosten per gewonnen QALY. De BIA zal de betaalbaarheid betreffen van HBOT t.o.v. standaardbehandeling vanuit het overheids- en verzekeringsperspectief. • TIJDPAD Maand 1-3: Voorbereiding Maand 4: Start patiënten inclusie Maand 18: Interim-analyse en besluit welke studiearmen doorgaan Maand 30: Einde patiënten inclusie Maand 42: Complete 1-jaars follow-up Maand 42-48: Statistische analyse, schrijven van manuscript en publicatie van resultaten. Maand 66: Verzamelen van 3-jaars follow-up en publicatie van langetermijnresultaten SUMMARY • OBJECTIVE(S)/RESEARCH QUESTION(S) Is Hyperbaric Oxygen Therapy (HBOT) (cost)effective in patients with ischaemic diabetic foot or leg ulcers (DFU)? • HYPOTHESIS HBOT improves tissue oxygenation and subsequently the healing of DFUs. Hereby major amputations (above the ankle) will be prevented. • STUDY DESIGN Multinational randomised clinical trial with an adaptive (multi-arm, multi-stage) design. • STUDY POPULATION(S)/DATASETS Patients with type I or II diabetes, peripheral vascular disease and an ulcer on their foot or leg. • INTERVENTION Patients allocated to the intervention group will undergo 20, 30, or at least 40 sessions of HBOT in addition to usual care, consisting of standard (wound) care and revascularization, if applicable. • USUAL CARE /COMPARISON Patients allocated to the control group will receive standard wound care and revascularization, if applicable. • OUTCOME MEASURES Primary outcome is the major amputation rate after 12 months follow-up. Secondary outcomes include cost effectiveness, amputation-free survival, minor amputation rate, complete ulcer healing and mortality. • SAMPLE SIZE CALCULATION/DATA ANALYSIS Up to 544 patients are needed to detect a 15% difference in major amputation rate. An interim analysis of 272 patients will determine which futile study arm(s) will stop and which promising ones continue. • COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS The economic evaluation will be performed as a cost effectiveness (CEA) and cost utility analysis (CUA). The primary outcome in the CEA will be costs per prevented amputation. The primary outcome in the CUA will be costs per QALY. In the budget impact analysis (BIA) the affordability of HBOT compared to standard wound therapy will be assessed from governmental and insurer perspectives. • TIME SCHEDULE Month 1-3: Trial preparation Month 4: Start patient inclusion Month 18: Interim data-analysis and decision about which study arms to continue Month 30: Patient inclusion completed Month 42: End of complete one-year follow-up Month 42-48: Statistical analysis, writing and publication of first results. Month 66: End of three-year follow up, collection of data.
