Eindverslag

Ongeveer 80% van de mensen ervaren tijdens hun leven een traumatische gebeurtenis, terwijl ongeveer 8% hiervan een posttraumatische stressstoornis (PTSS) ontwikkelt. Indien de PTSS onbehandeld blijft, heeft de aandoening gewoonlijk een chronisch, aanhoudend verloop, en leidt deze tot aanzienlijke beperkingen in sociaal functioneren. Sociale steun is één van de meest krachtige beschermende factoren in het verminderen van het risico op de ontwikkeling van PTSS en het verhogen van de kans op het herstel van PTSS. Op dit moment is er echter weinig bekend over de neurobiologische mechanismen die hieraan ten grondslag liggen. Eerder onderzoek heeft aangetoond dat PTSS geassocieerd is met een verhoogde activiteit van het sympathische zenuwstelsel (SNS) en met een ontregelde hypothalamus-hypofyse-bijnier (HPA)-as. Van sociale steun is bekend dat het stress-gerelateerde activiteit in SNS en HPA-as kan verminderen, en dat het een positief effect heeft op de geestelijke en lichamelijke gezondheid. Bij patiënten met PTSS is verhoogde activiteit gevonden in de amygdala, een hersengebied dat centraal staat in de biologische reactie op gevaar. Daarnaast is PTSS geassocieerd met een verminderd vermogen om beloning te ervaren. Emotionele verdoofdheid, een belangrijk symptoom van PTSS, kan het gevolg zijn van problemen in de beloningsmechanismen in de hersenen. Dit impliceert dat het feit dat mensen met PTSS moeite hebben met het opzoeken of inzetten van sociale steun niet alleen gebaseerd is op het vermijden van angst reacties, maar ook een ongevoeligheid voor sociale beloningen zou kunnen weerspiegelen. Wij hypothetiseren dat het "sociaal verbindende" hormoon oxytocine een belangrijke rol zou kunnen spelen in de vermindering van de negatieve reacties op trauma door het reguleren van angst en stress reacties, en door het verbeteren van sociale interactie door de gevoeligheid voor beloning te vergroten. In een aantal studies onderzoeken wij de rol van oxytocine in de stressrespons en de onderliggende neurobiologische mechanismen in mensen die kort geleden een traumatische gebeurtenis hebben meegemaakt en in patiënten met PTSS. In een serie van experimenten onderzoeken we het effect van oxytocine op de hersenactiviteit door middel van functionele beeldvorming (fMRI) na endogene (het verstrekken van sociale steun) of exogene (intranasale oxytocine administratie) stimulatie van het oxytocine-systeem in getraumatiseerde mannen en vrouwen met en zonder PTSS. De effecten van oxytocine op de hersenactiviteit worden onderzocht in relatie tot psychosociale- en gedragsveranderingen (angst, stemming) en psychofysiologische en neuro-endocriene reacties. Daarnaast worden de mogelijke modererende effecten van geslacht, jeugdtrauma en hechtingsstijl onderzocht. De resultaten zullen unieke gegevens opleveren over de rol van oxytocine in psychobiologische reacties op trauma, en zullen de kennis over de fundamentele mechanismen die ten grondslag liggen aan de ontwikkeling van PTSS vergroten. Dit is essentieel voor het voortbrengen van innovatieve interventies zoals de toepassing van oxytocine bij het voorkómen of behandelen van trauma-gerelateerde aandoeningen.
