Morbidity and mortality rates caused by cardiovascular diseases are very high in deprived neighbourhoods. Multiple risk factor interventions targeted at patients with a high-risk for cardiovascular diseases are beneficial. Secondary prevention strategies in general practice based on GP guidelines could be more effective. Due to the high workload of GPs in deprived neighbourhoods extra personnel is needed to organise this preventive care more (cost-)effectively. The proposed randomised controlled trial evaluates the cost-effectiveness of intensification of preventive care in patients with a 10-years absolute risk for cardiovascular diseases that is higher than 20% by supplying a practice nurse and a migrant health educator to the practice. The cost-effectiveness is based on the decrease in absolute risk in one year and on all medical and non-medical, direct and indirect costs. Because randomisation will take place within one general practice, information about implementation facilitators and obstacles will be collected.
