Measuring cannabis use, abuse and dependence across different cultural settings

Dit onderzoeksproject richt zich op het onderzoeken of criteria voor cannabis misbruik en cannabis afhankelijkheid voor verschillende groepen individuen wel in vergelijkbare mate gelden (bijvoorbeeld voor vrouwen en voor mannen, voor jonge gebruikers versus oudere gebruikers). Daarbij ligt een specifieke focus op de vergelijking tussen Amerikaanse cannabis gebruikers en Nederlandse cannabisgebruikers, vanwege de verschillen in beleid tussen beide landen, en wordt er ook veel aandacht besteed aan de culturele herkomst van cannabis gebruikers. Hiertoe wordt een nieuwe Nederlandse dataset verzameld bij jongeren met een niet-westerse afkomst. Tot slot zullen we met behulp van genetisch informatieve samples kijken naar de genetische en omgevingsfactoren die een rol spelen bij de verschillende symptomen van cannabis misbruik en afhankelijkheid. De vraagstellingen uit dit onderzoeksproject worden beantwoord aan de hand van bestaande datasets en aan de hand van een nieuwe, unieke dataset waarvoor Nederlandse jongeren (15-24 jaar) uit verschillende herkomstgroeperingen geworven worden. De dataverzameling van dit project, genaamd i4culture, verloopt goed en zal binnen enkele maanden worden afgerond. Twee papers worden binnenkort ingediend bij internationale wetenschappelijke tijdschriften.

Samenvatting van de aanvraag

Research concerning the use of cannabis and cannabis use disorders is hampered by the lack of valid and reliable criteria. The current DSM-IV-TR criteria have severe limitations, particularly regarding the cultural context in which cannabis use takes place, and the age and sex of cannabis users. The functioning of DSM-IV-TR criteria for cannabis abuse and dependence is considered largely invariant from a cultural perspective, particularly in Western cultures. However, there is little empirical evidence to support this assertion. The current grant application proposes an innovative study of a combined international sample (Dutch, U.S., Australian), bringing together a team of cannabis epidemiologists and geneticists, to comprehensively investigate the role of cultural context in the functioning of DSM-based assessments of cannabis use and dependence. Of particular interest here is the role of ‘semi-legal/illegal’ status of cannabis in the Netherlands versus the U.S. and Australia and how this potentially impacts interpretation of DSM criteria, and furthermore, whether variations in cohort, gender, comorbid psychopathology induce differential criterion functioning. By combining several large scale datasets including both adolescent and young adult singletons from the Netherlands, the US, and twins from the U.S. and Australia, we will address several measurement-related issues in the etiology of cannabis use disorders. The ultimate goal of this proposed multi-cultural, multi-ethnicity international collaborative study is to come to phenotypic refinement of cannabis use, abuse and dependence definitions. It will result in a focused and group-specific instrument to assess cannabis use disorders in adolescents and young adults.
