Safe and Timely Cessation of Isolation of Patients with COVID-19, The SCIP-COVID-19 study

De duur van besmettelijkheid van COVID-19 en dus de duur van isolatie tijdens de ziekenhuisopname is nog niet bekend. Te vroeg uit isolatie geeft besmettingen in het ziekenhuis. Te laat uit isolatie is erg belastend voor de patiënt. De onderzoekers van deze studie denken dat de isolatieperiode korter kan. Alleen is er niet voldoende kennis hoe vast te stellen wanneer iemand niet meer besmettelijk is. Daarom wordt in deze studie een aantal kenmerken bij elkaar genomen, die aan- of afwezig moeten zijn voordat je kan zeggen; “deze patiënt is niet meer besmettelijk”.

Doel

Het doel van deze studie is om aan de hand van een aantal kenmerken te bepalen waneer een patiënt niet meer besmettelijk is bij een corona-infectie en uit isolatie kan.

Onderzoeksopzet

Er is al veel informatie over de patiënten verzameld. Deze worden hergebruikt en aangevuld om de vraag van deze studie te beantwoorden.

Te verwachten resultaten

De resultaten van het onderzoek worden door ziekenhuizen gebruikt en internationaal verkondigd.

Context

Dit onderzoek is 1 van de studies op het gebied van onderzoek naar optimale medisch-specialistische zorg bij COVID-19. De onderzoeken hebben als doel het beantwoorden van belangrijke kennishiaten op gebied van behandeling. ZonMw faciliteert deze onderzoeken in het COVID-19 deelprogramma Behandeling om een bijdrage te leveren aan het bevorderen van optimale zorg voor COVID-19. De kennishiaten zijn geïdentificeerd door de Multidisciplinaire Wetenschapscommissie COVID-19 van de Federatie Medisch Specialisten (FMS).

Samenvatting van de aanvraag

Research question There is an urgent need for defining of the period of infectiousness. We will develop a safe and effective algorithm for decision on timely end isolation of COVID-19 hospitalized patients. The SCIP-COVID-19 project ensures an efficient use of the scarce number of isolation beds for hospitalized patients, while maintaining the highest and safest standards of care. Hypothesis We hypothesize that the isolation duration of COVID-19 patients is too long, which is highly undesired for patients and HCW. Study design work packages (WP)1: Retrospective cohort study of admitted COVID-19 patients in 7 hospitals, and selected data on positive HCW. WP2: Technical laboratory study using stored samples for serology and for Ag-RDT to find the value of adding these to the algorithm. VOCs and patient characteristics on infectiousness will be taken into account. WP3: Formulating new criteria for isolation cessation. Study population All COVID-19 patients of the participated hospitals. Intervention NA. Outcome measures WP1: A reusable database of > 7000 COVID-19 patients, an overview of COVID-19 isolation measures over time. WP2: Enrichment of the database, platform-specific Ct value cut-offs for non-infectious viral RNA detection and an evidence-based decision for serology and Ag-RDT tests (qualitative and quantitative) as part of the algorithm. WP3: a set of criteria to end isolation, implementation together with patient organizations. Sample size / data analysis For WP1, we aim to include >7000 patients. For WP2, we base our sample size calculation on the primary outcome analysis, i.e. the comparison of the mean Ct-value of the RT-PCR as reference and the quantitative Ag-concentration of the Ag-test. Assuming a correlation as small as -0.20 between the (log transformed) continuous Ct-value and the quantitative Ag-test concentration, a sample size of N=250 patients is sufficient to estimate this correlation with a power >0.90 and two-sided alpha 0.05.
