Risk of Zika virus introductions for the Netherlands (ZikaRisk)

Het Zikavirus (ZIKV) is een virus dat wordt overgebracht door steekmuggen op mensen. Normaliter veroorzaakt een ZIKV infectie bij mensen zika-koorts die gepaard gaat met milde ziekteverschijnselen, maar kan ook gepaard gaan met een ernstiger ziektebeeld. Zo kan de infectie ook leiden tot het zeldzame Guillain-Barre syndroom en er kan hersenschade in het ongeboren kind optreden wanneer de moeder tijdens de zwangerschap met ZIKV geïnfecteerd is geraakt.

Doel

Sinds eind 2015 is er een grootschalige uitbraak van ZIKV geweest in Zuid- en Midden-Amerika en in het Caribisch gebied. Destijds werd vooral zwangere vrouwen afgeraden om die gebieden te bezoeken. Inmiddels is het risico op een ZIKV infectie in die gebieden afgenomen. Maar hoe groot is de kans dat ZIKV in Nederland geïntroduceerd wordt?

In het ZikaRisk project werken het Erasmus Medisch Centrum (EMC) en Wageningen Universiteit (WU) samen om het ziekterisico van ZIKV voor verschillende bevolkingsgroepen in Nederland te onderscheiden en de kans op ZIKV verspreiding in Nederland en Noord-Europa in te schatten. Dit wordt gedaan door middel van het vaststellen van nieuwe (incidentie) en bestaande (prevalentie) gevallen van ZIKV en andere arbovirussen bij Nederlandse burgers, proefdiervrije modellen en virusoverdracht-experimenten met Nederlandse muggen. De resultaten van dit project zijn van belang om het risico van ZIKV in Nederland en Noord-Europa te bepalen en de bevindingen te vertalen naar een generaliseerbare toolbox voor risicokarakterisering van nieuwe arbovirussen.

Resultaten

Uit de resultaten van het project blijkt dat het risico om in Nederland een Zika virusinfectie op te lopen erg klein is. Het is wel mogelijk dat een Zika infectie versterkt wordt als er zich antistoffen tegen gerelateerde virussen in het lichaam bevinden via een eerdere besmetting of vaccinatie. Zwangerschap lijkt dit versterkende effect niet te beïnvloeden. Verder is er aangetoond dat exotische muggen, zoals Aziatische bosmug die in Nederland voorkomt, in staat zijn het Zika virus te verspreiden. De huissteekmug kan dit echter niet. Ook is er een toolbox met nieuwe serologische (bloedonderzoek) en virologische (moleculair onderzoek) testen ontwikkeld om het risico van de nieuwe arbovirussen te karakteriseren.

Samenvatting van de aanvraag

Zika virus (ZIKV), a mosquito-borne flavivirus previously considered a zoonosis of limited importance to human health, is causing an epidemic of unprecedented scale in Middle/South America and the Caribbean. Urban ZIKV is transmitted in a cycle involving humans and mosquitoes, but maintenance in major areas of the world has involved non-human reservoir hosts. Previously undetected severe clinical complications (microcephaly, Guillain-Barre syndrome) form a major health concern, yet are poorly understood. A One Health approach involving medical/veterinary/insect virologists and medical entomologists is essential 1) to determine the risk of disease in ZIKV-exposed patients in The Netherlands including the overseas territories, 2) to understand the explosive spread of ZIKV and possible introduction/establishment in the Netherlands, and 3) to allow risk assessment for ZIKV and other future arboviral outbreaks. We will determine the risk of ZIKV establishment based on prospective studies in travellers and vector competence studies in indigenous mosquitoes, and develop animal-free laboratory models to study ZIKV tropism and disease.
