Data Driven Essential Care in District Nursing: improving patient outcomes and maintaining independence

More than half a million people in the Netherlands receive district nursing care daily. 

Aim

To improve quality, the DataNurse project developed two innovations: a shared decision-making preparatory tool and a data dashboard. 

Results

  • The shared decision-making preparatory tool supports district nurses and clients in better conversations about personal goals and independence. Clients valued this approach and conversations became more meaningful. 
  • The dashboard makes care data visible, but learning from data requires time, training, and a culture in which data is seen as valuable. 

Conclusions and next question

The feasibility study showed that both innovations are promising, but that success depends on time and continuity. Conducting research in district nursing also proved challenging: teams struggled to combine participation with daily care practice. DataNurse shows what is possible. The next question is how shared decision-making and learning from data can become a natural part of education and the daily work of nurses and care assistants.
