INTER-ACT: Roadmap for interprofessional collaboration in nutrition and mobility in the hospital

In hospitals, allied health professionals and nurses often work within fragmented structures. For patients with complex conditions, this can lead to suboptimal nutritional and mobility care like malnutrition, muscle dysfunction and reduced mobility. Not only does this delay patients’ recovery, but it could also lead to an increased risk of complication, longer hospitalization, higher costs and lower self-management and quality of life.

Aim

This project aims to improve and evaluate interprofessional collaboration among nurses, physiotherapists and dietitians regarding nutrition and physical activity.

Approach

Across 3 university hospitals, we will develop a practical roadmap with well-defined steps to enhance and sustainably implement this collaboration. The roadmap includes clear guidelines for roles, processes, responsibilities and the use of an integrated nutrition and activity plan.
It will also be applied within 3 existing initiatives, and outcomes will be assessed at patient, professional and organizational levels to advance high-value care, prevention, recovery, and job satisfaction.
