PASSION: Interprofessional collaboration for appropriate care in ABI and frail older adults

People with complex care needs, such as frail older adults and individuals with acquired brain injury (ABI), often receive support from multiple professionals across healthcare and the social domain. However, limited coordination between these professionals results in duplication of services and puts an additional burden on patients and informal caregivers.

Aim

In this project, we investigate how effective interprofessional collaboration between nurses, allied health professionals, medical specialists, general practitioners and social workers contributes to appropriate, integrated care. 

Approach

Through questionnaires, interviews, and observations we aim to identify effective elements and bottlenecks in interprofessional collaboration. These insights are then directly translated into concrete tools and practical approaches, enabling teams to strengthen their collaboration and ensuring that results can be rapidly applied in practice, policy and education.
