Projectomschrijving

Atopisch eczeem is een veelvoorkomende chronische huidaandoening. Patiënten kunnen, afhankelijk van de ernst van de ziekte, behandeld worden met lokale therapie (zalven), lichttherapie of systemische therapie.

Lichttherapie wordt vaak gebruikt als tussenstap tussen lokale en systemische therapie bij patiënten met onvoldoende ziektecontrole door lokale therapie. Lichttherapie wordt regelmatig voorgeschreven.
Echter, het bewijs voor de effectiviteit van lichttherapie is enkel in korte en laag tot matige kwaliteit gerandomiseerde studies onderzocht. Er is onderzoek van hoge kwaliteit nodig om meer inzicht in de (kosten)effectiviteit van lichttherapie te krijgen. Dit heeft geleid tot één van de kennisvragen van onze beroepsvereniging (NVDV).

Dit project heeft als doel om de (kosten)effectiviteit van de meest gebruikte vorm van lichttherapie (narrowband UVB) bij patiënten met atopisch eczeem te onderzoeken. Dit wordt onderzocht met een multicenter gerandomiseerde studie in Nederland.

Samenvatting van de aanvraag

DOEL: Het vergelijken van effectiviteit en kosteneffectiviteit van narrowband ultraviolet B met optimale topicale therapie (NB-UVB+OTT) versus OTT op 3 maanden bij volwassen patiënten (ptn) met atopisch eczeem (AE). HYPOTHESE: Een NB-UVB+OTT kuur van minimaal 8 weken is superieur aan OTT in termen van reductie van ziekte-ernst op 3 maanden. NB-UVB+OTT leidt tot uitstel van systemische therapie bij follow-up van 1 jaar. PATIENT (P): Volwassen AE ptn met onvoldoende controle van hun AE met topicale therapie, waarvan dermatoloog en pt samen van mening zijn dat NB-UVB of OTT nodig is. INTERVENTION (I): Een NB-UVB+OTT kuur (thuis of poliklinisch) van minimaal 8 en maximaal 16 weken. COMPERATOR (C): OTT. OUTCOME (O): AE core outcome set uitkomstmaten worden gebruikt. Primaire uitkomstmaat: % ptn met EASI50 (Eczema Area and Severity Index) op 3 maanden follow up. Secundaire uitkomstmaten: Delta EASI, arts en pt gerapporteerde symptomen, kwaliteit van leven, ziekte controle, kosteneffectiviteit en bijwerkingen op 1-3-6-9-12 maanden. Hoeveelheid en potentie topicaal steroïd gebruik, tijd tot overgaan naar systemische therapie en pt tevredenheid met ontvangen therapie. % ptn dat Treatment Target Goals behalen en % uitval met redenen op 3-6-9-12 maanden. FOLLOW-UP TIME: Follow-up tot 1 jaar na start NB-UVB+OTT. De bezoeken zullen plaatsvinden op baseline en 1-3-6-9-12 maanden na start. Extra visite(s) bij switch of stoppen therapie. STUDIE OPZET: Pragmatische, multicentrische, enkelblinde gerandomiseerde gecontroleerde trial gekoppeld aan prospectief observationeel cohort van het TREAT NL (TREatment of ATopic eczema, the Netherlands) register. SAMPLE SIZE BEREKENING/DATA-ANALYSE: Per studiegroep zijn 134 patiënten nodig om een absoluut verschil van 20% in de primaire uitkomstmaat tussen de NB-UVB+OTT (50%) en OTT groep (30%) te detecteren (power 90%, 2?zijdige alpha 0.05). Rekening houdend met 15% uitval resulteert dit in totaal 316 ptn. De primaire analyse betreft een ‘intention to treat’ analyse van het verschil in de primaire uitkomstmaat tussen beide groepen. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE: Volledige economische evaluatie van NB-UVB+OTT vs OTT vanuit maatschappelijk perspectief met een tijdshorizon van 1 jaar. Een BIA vanuit perspectieven van overheid, zorgverzekeraar en zorgaanbieder. TIJDPAD: Totale duur 4 jaar. Opstart: 9 mnd. Inclusieperiode: 1.5 jaar. Follow-up: 1 jaar. Analyse: 9 mnd. OBJECTIVE(S): Comparing the effectiveness and cost-effectiveness of narrowband ultraviolet B with optimal topical therapy (NB-UVB+OTT) versus OTT at 3 months in adult patients (pts) with atopic eczema (AE). HYPOTHESIS: A NB-UVB+OTT course of at least 8 weeks is superior to OTT alone in decreasing disease severity at 3 months. NB-UVB+OTT will delay systemic therapy during follow-up of 1 year. PATIENT (P): Adult AE pts with insufficient control of their AE with topical treatment, for whom dermatologist and patient jointly believe that NB-UVB or OTT is needed. INTERVENTION (I): A NB-UVB+OTT course (home or outpatient) for at least 8 and up to 16 weeks. COMPARATOR (C): OTT. OUTCOME (O): AE Core Outcome Set outcome measures will be used. Primary outcome measure: % pts with EASI50 (Eczema Area and Severity Index) at 3 month follow-up. Secondary outcome measures: Delta EASI, physician and pt reported symptoms, quality of life, disease control, cost effectiveness and side effects at 1-3-6-9-12 months. Quantity and potency topical steroid usage, time to starting systemic therapy and pt satisfaction with received treatment. % pts reaching Treatment Target goals and % drop-outs with reasons at 3-6-9-12 months. FOLLOW-UP TIME: Follow-up time for 1 year after NB-UVB+OTT start. Visits will occur at baseline and 1-3-6-9-12 months after start. Extra visit(s) will take place in case of therapy switch or stop. STUDY DESIGN: Pragmatic multicenter single blinded randomized controlled trial linked to prospective observational cohort in the TREAT NL registry. SAMPLE SIZE CALCULATION/DATA ANALYSIS: Per study group 134 pts are needed to detect an absolute difference of 20% in the occurrence of the primary outcome measure between the NB-UVB+OTT (50%) and OTT (30%) groups (power 90%, 2-sided alpha 0.05). To allow a dropout rate of 15% a total of 316 pts need to be included. The main analysis will concern ‘intention to treat’ analysis of the difference in the primary outcome measure. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS: Full economic evaluation of NB-UVB+OTT vs OTT alone from a societal perspective and 1 year time horizon. Budget impact analysis from governmental, health insurer and provider perspectives. TIME SCHEDULE: Total duration: 48 months. Start-up: 9 months. Inclusion period: 1.5 years. Follow-up: 1 year. Analysis: 9 months.
