Self-Care@Home: hybrid and digital self-care support for older people with multimorbidity
An increasing number of older people live at home and have several diseases. Symptoms of their diseases often fluctuate, limiting them in their activities and leading to frequent hospital visits. This challenges older people, their families, and the healthcare system.
Aim
With the Self-Care@Home research line, we aim to improve care and independence for these older people.
Approach
We do this by supporting self-care that fits what older people find important. Therefore, we study how hybrid and digital self-care, supported by nurses, can support older people in better managing their symptoms. This may reduce symptom burden and hospital visits while improving the ability to live independently at home for longer. We adapt and test hybrid and digital self-care tools with older people, nurses, and doctors. We study for whom and in what situations these tools work best. By doing so, we enable Appropriate Care and strengthen the role of nurses and nursing science
